Amerisure Mutual Holdings, Inc. (including its affiliates, which are collectively known as “Amerisure” or the “Amerisure Companies”) value the trust and confidence of our policyholders and others with whom we do business. With this in mind, we have to explain our privacy practices in this Privacy Notice (this “Notice”).
This Notice describes the types of information we may collect from you or that you may provide to us when you utilize our services or visit this website, and our practices for collecting, using, maintaining, protecting, and disclosing that data.

- Through our services and business processes, including – but not limited to – underwriting, financial data processing, administering policies, and claims handling.

CHILDREN UNDER THE AGE OF 13
This website and our services are not intended for children under 13 years of age. No one under age 13 may provide any personal information to us or on this website. We do not knowingly collect personal information from children under 13. INFORMATION WE COLLECT ABOUT YOU AND HOW WE COLLECT IT
We collect several types of information during the course of underwriting, administering an insurance policy, handling a claim, or otherwise utilizing this website and Amerisure’s services, Amerisure may collect certain information by which you may be personally identified.
Personal information may be collected from persons such as Amerisure’s independent insurance agents or brokers, a policyholder, a claimant, a claimant’s employer (in the case of a workers’ compensation claim), persons who may have witnessed an incident related to a claim, or persons retained by the claimant or by us in the process of administering a policy or adjusting a claim, such as repair shops, physicians, attorneys, or accountants. We may collect this data directly from you, when provided to us; automatically, as you navigate through our products and services or our online website; or from third parties we are associated with, such as business partners, affiliates, and subsidiary companies.
Information You Provide to Us
The information we collect on or through our products, services, and website may include, among other information: individuals names; contact information such as home and work addresses and phone numbers; federal tax identification and social security numbers; policy-related information such as policy numbers, policy premiums and payment histories; claim numbers, loss-related information, and claim-related information like claim numbers, status, and value.
We may also collect the following data:
- Information that you provide by filling in forms on our website or through our products and services. This includes information provided at the time of registering to use our website, purchasing an Amerisure policy, or requesting further information, products, or services from us.
- Information that you provide in order to apply for a job with or in partnership with Amerisure.
- Your responses to surveys that we might ask you to complete for research and development, innovation, quality control, and other purposes.
- Data related to identifiers you may use online or offline that are about you but do not identify you directly.
- Information you provide when you report a problem with our products, services, or this website.
- Your search queries on our website.
- Records and copies of your correspondence (including email addresses), if you contact us, as well as any agreements or policies you may have with Amerisure.
Information We Collect Through Automatic Data Collection Technologies
As you navigate through and interact with our website, and utilize Amerisure’s products and services, we may use automatic data collection technologies to collect certain information about your equipment, browsing actions, and patterns, including:
- Details of your visits to our website, such as traffic data, location data, logs and other communication data and the resources that you access and use on our website or through our products.

We do not collect personal information automatically, but we may tie non-personal information collected automatically to personal information about you that we collect from other sources or that you provide to us.
THIRD-PARTY USE OF COOKIES AND OTHER TRACKING TECHNOLOGIES
Some parts of this website may be served by third-parties. These third parties may use cookies alone or in conjunction with web beacons or other tracking technologies to collect information about you when you use our website or our services. Moreover, Amerisure may share your information with affiliated and non-affiliated third parties to the extent necessary to service an insurance policy or claim, or as permitted or required by law. Third parties with whom non-public personal information may be shared for these purposes include our independent insurance agents and brokers, policyholders, persons retained to assist with the effective and efficient administration of policies and claims, persons retained to implement, develop, and grow effective and efficient administration of policies and claims, and others as may be permitted by law.
DISCLOSURE OF YOUR INFORMATION
We may disclose aggregated information about our users, and information that does not identify any individual, without restriction. While we generally keep personal information confidential, where relevant we may disclose personal information that we receive from you:
- To our subsidiaries and affiliates.
- To nonemployee contractors, service providers, and other third parties we use to support our business, such as IT vendors, actuaries, auditors, lawyers, document management providers, outsourced business process management providers, tax advisers, and third parties who assist with our direct marketing activities.
- To a buyer or other successor in the event of a merger, divestiture, restructuring, reorganization, dissolution, or other sale or transfer of some or all of Amerisure’s assets, whether as a going concern or as part of bankruptcy, liquidation, or similar proceeding, in which personal information held by Amerisure about our website users, clients, and customers is among the assets transferred.

CHOICES ABOUT HOW WE USE AND DISCLOSE YOUR INFORMATION
We provide you with choices regarding the personal information you provide to us. We have created mechanisms to provide you with the following control over your information:

- Promotional Offers from the Company. This opt-out feature does not apply to information provided by Amerisure to you as a result of a services purchase, product service experience, policy processing, claims handling, or other transactions.

ACCESSING AND CORRECTING YOUR INFORMATION
You can review, update, and correct your personal information by logging into the website and visiting your account page.
You may also reach out to our Office of General Counsel at the contact information listed below to request access to, correct, or delete any personal information that you have provided to us. We may not accommodate a request to change information if we believe the change would violate any law or legal requirement or cause the information to be incorrect.

YOUR STATE PRIVACY RIGHTS
California, Colorado, Connecticut, Delaware, Florida, Indiana, Iowa, Montana, Oregon, Tennessee, Texas, Utah, and Virginia provide (now or in the future) their state residents with rights to:
- Confirm whether we process their personal information.
- Access and delete certain personal information.
- Correct inaccuracies in their personal information, taking into account the information’s nature processing purpose (excluding Iowa and Utah).
- Data portability.
- Opt-out of personal data processing for: targeted advertising (excluding Iowa); sales; or profiling in furtherance of decisions that produce legal or similarly significant effects (excluding Iowa and Utah).

Nevada provides its residents with a limited right to opt-out of certain personal information sales. Residents who wish to exercise this sale opt-out rights may contact our Office of General Counsel for additional information. However, please know we do not currently sell data triggering that statute’s opt-out requirements.

DATA SECURITY
We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, and disclosure. The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our website, products, or services, you are responsible for keeping this password confidential. We ask you not to share your password with anyone. We urge you to be careful about giving out information in public areas of the website. The information you share in public areas may be viewed by any user of the website.
Unfortunately, the transmission of information via the internet is not completely secure. Although we do our best to protect your personal information, we cannot guarantee the security of your personal information transmitted to our website, or via our online services. CALIFORNIA PRIVACY NOTICE SUPPLEMENT
This section of the Notice provides supplemental information relevant under California law (this “Supplement”) and applies only to individuals who reside in the State of California. We adopt this Supplement to the Notice in compliance with the California Consumer Privacy Act of 2018 (“CCPA”) and other California privacy laws. The terms used in this Supplement, and which are also defined by the CCPA, are intended to have the same meaning.
Information Collected by Amerisure
In order to provide quality products and services, we may require access to certain personal information, as disclosed in this Notice. Amerisure collects information that identifies, relates to, describes, references, and is capable of being associated with, or could reasonably be linked, either directly or indirectly, with a particular consumer or household. Within the last 12 months, Amerisure has collected the following categories of personal information from consumers:

Amerisure obtains the categories of personal information identified in (A) through (K) above from the source categories disclosed in INFORMATION WE COLLECT ABOUT YOU AND HOW WE COLLECT IT above.
Use of Your Personal Information
Amerisure considers the personal information collected to be confidential. However, Amerisure may use or disclose the personal information we collect for one or more of the business purposes disclosed in HOW WE USE YOUR INFORMATION. Amerisure does not collect any categories of personal information or use your personal information for any materially different or related purposes without providing you with notice.
Sharing of Your Personal Information
Amerisure requires that employees having access to personal information of another person take special precautions to protect the confidentiality of that personal information. For more information on how personal information is shared by us, see DISCLOSURE OF YOUR INFORMATION.
In the preceding 12 months, Amerisure has disclosed the above referenced categories of personal information for a business purpose or other purposes, but has not sold any such personal information.
Your Rights and Choices under the CCPA
The CCPA provides California residents with specific rights regarding their personal information. California residents may exercise their CCPA Rights and Choices with Amerisure as follows:
Right to Receive Information on Privacy Practices
You have the right to receive the following information from us at or before the point of collection: (1) the types of personal information to be collected; (2) the purposes for which the types of personal information are collected or utilized by Amerisure; (3) disclosure regarding whether or not that personal data is sold or shared with others; (4) if Amerisure the collects sensitive personal information, the types of sensitive personal information to be collected, the purposes for which it is collected or utilized, and whether that information is sold or shared with others; and (5) the time period Amerisure intends to retain each type of personal information, or if that is not possible, the criteria used to determine that length of time.
Access to Specific Information and Data Portability Rights
You have the right to request that Amerisure disclose certain information to you about Amerisure’s collection and use of your personal information over the past 12 months. Once Amerisure receives and confirms your verifiable consumer request for personal information, we will disclose to you:
- The categories of personal information we collected about you.
- The categories of sources from which the personal information was collected.
- Our business or commercial purpose for collecting that personal information.
- The categories of third parties with whom we share that personal information.
- The specific pieces of personal information we collected about you.
- If we disclosed your personal information for a business purpose the categories of personal information disclosed and to whom.
Deletion Request Rights
You have the right to request that Amerisure delete any of your personal information that Amerisure collected and retained about you, subject to certain exceptions. Once we receive and confirm your verifiable consumer request for deletion, we will delete (and direct our service providers to delete) your personal information from our records, unless an exception applies.
Amerisure may deny your verifiable consumer request for deletion if retaining the information is necessary for us or our third-party service providers to:
- Complete the transaction for which we collected the personal information, provide a good or service that you requested, take actions reasonably anticipated within the context of our ongoing business relationship with you, or otherwise perform our contract with you.
- Detect security incidents, protect against malicious, deceptive, fraudulent, or illegal activity, or prosecute those responsible for such activities.
- Debug products to identify and repair errors that impair existing intended functionality.
- Exercise free speech, ensure the right of another consumer to exercise their free speech rights, or exercise another right provided for by law.
- Comply with the California Electronic Communications Privacy Act (Cal. Penal Code § 1546 seq.).
- Engage in public or peer-reviewed scientific, historical, or statistical research in the public interest that adheres to all other applicable ethics and privacy laws, when the information’s deletion may likely render impossible or seriously impair the research’s achievement, if you previously provided informed consent.
- Enable solely internal uses that are reasonably aligned with consumer expectations based on your relationship with us.
- Comply with a legal obligation.
- Make other internal and lawful uses of that information that are compatible with the context in which you provided it.
Exercising Access, Data Portability, and Deletion Rights
To exercise your access, data portability, and deletion rights, you may submit a verifiable consumer request to us by calling us or emailing us at the contact information listed in the CONTACT INFORMATION section of this Notice.
You or a person registered with the California Secretary of State who is authorized to act on your behalf, may make a verifiable consumer request related to your personal information. You may also make a verifiable consumer request on behalf of your minor child. You may only make a verifiable consumer request twice within a 12-month period and your request must:
- Provide sufficient information to enable Amerisure to reasonably verify that you are the person about whom we collected personal information or that you are an authorized representative of that individual; and,
- Provide sufficient information and detail which enables Amerisure to understand, evaluate, and respond to your request.
We cannot respond to your request or provide you with personal information if Amerisure cannot reasonably verify that the request was made by you, by you on behalf of your minor child, or by a person registered with the California Secretary of State who is authorized to act on your behalf or the behalf of your minor child. You will not be required to create an account with Amerisure to make a verifiable consumer request. Personal information that may be provided within a verifiable consumer request will only be used verify the requester’s identity and request authority.
Response Time and Format of Response
Amerisure strives to respond to all verifiable consumer requests within 45 days of receipt. However, we may need more time to respond to your request – such as an additional 45 days to respond to and deliver the information you requested to you. If additional response time is needed, Amerisure will inform you of the reason for delay, and the additional amount of time Amerisure will need to respond to your request (not to exceed 90 days from Amerisure’s receipt of your initial verifiable consumer request). If you have an account with us, the response will be delivered in writing to you at that account. If you do not have an account with us, we will deliver the response to you in writing to you by mail, or electronically, as you may choose to elect.
Amerisure’s response to your verifiable consumer requests will cover the 12-month period preceding Amerisure’s initial receipt of your verifiable consumer request. The response will be provided in a readily usable format so that your personal information can be transmitted by you from one entity to another without hindrance.
A fee will not be charged in processing or responding to a verifiable consumer request unless such request is excessive, repetitive, or manifestly unfounded. If such a request is made, and it is determined that a fee will be required for any of the reasons stated above, Amerisure will inform you of the same and provide you with a cost estimate before completing your request.
Non-Discrimination by Amerisure
Amerisure does not discriminate against California residents who exercise their rights under the CCPA, and unless permitted by the CCPA, Amerisure will not:
- Deny you goods or services.
- Charge you different prices or rates for goods or services, including through granting discounts or other benefits, or imposing penalties.
- Provide you a different level or quality of goods or services.
- Suggest that you may receive a different price or rate for goods or services or a different level or quality of goods or services.
Changes to Our Privacy Notice
Amerisure may amend or change this Notice, including this Supplement, from time to time, as provided in the CHANGES TO OUR PRIVACY NOTICE section. You agree that you are responsible for reviewing these Notice regularly, and your continued use of this website or any other Amerisure website constitutes your acceptance of any modifications, changes, or revisions to Amerisure’s online services or this Notice.
